Accessibility Statement
Forge Physical Therapy is committed to making our website and our clinic accessible to everyone, including people with disabilities. We aim to meet or exceed the Web Content Accessibility Guidelines (WCAG 2.1 Level AA) and we treat accessibility as an ongoing effort, not a one-time project.
Our ongoing work
We regularly review our site to identify and address accessibility barriers. As we update content, add new features, and bring new pages online, we test compatibility with common assistive technologies and screen sizes.
Assistive technologies we support
Our site is designed to work with widely used assistive technologies, including:
- Screen readers such as JAWS, NVDA, and VoiceOver
- Keyboard-only navigation
- Screen magnifiers
- Voice-control software
Known limitations
Some content on our site is provided by third-party vendors — for example, our online booking tool (Jane App) and our embedded map (Google Maps). We select vendors with accessibility in mind and work with them when issues arise, but we cannot guarantee full conformance for content we do not control.
If third-party content is preventing you from completing a task — for example, scheduling an appointment — please contact us and we will help you directly.
Requesting an accommodation
If you need an accommodation to use our services — such as an alternate format for information on our site, help scheduling an appointment, or an adjustment to how we communicate with you — please let us know:
- Phone: (214) 774-0600
- Email: admin@forgephysicaltherapy.com
We aim to respond within two business days.
Reporting an accessibility issue
If you encounter a barrier on our website, please tell us so we can fix it. Helpful details include:
- The page URL where you saw the issue
- A description of the problem
- The browser and assistive technology you were using
Email us at admin@forgephysicaltherapy.com or call (214) 774-0600. Your feedback helps us prioritize fixes.
Formal complaints
In addition to contacting us, you have the right to file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, regarding accessibility in healthcare.
