Most people come to us after something else hasn't worked. A surgeon said "rest it." A generalist clinic had you on the same three machines every visit. Your evaluation is 60 minutes with Dr. Spencer. Follow-up visits are typically 45–60 minutes, directed by Dr. Spencer — hands-on techniques, skilled exercise progression, and in-session reassessment. Every visit is run by your DPT, not by a rotating cast of aides.

Evidence-based, not dogma-based
We don't push a single method, tool, or philosophy. Treatment is drawn from what the current research supports for your specific condition and presentation — manual therapy, dry needling, targeted exercise, motor control retraining, and patient education, used when and where they earn their place.

Every plan of care starts with a clear goal (what you want to do again without pain) and objective markers we can measure (range, strength, symptom behavior, functional tests). You will know what progress looks like — and you will know when you're done.
What to expect
Your first visit, start to finish
Before you arrive
You'll complete a short intake through Jane App (our booking and records system). We verify your insurance and confirm your copay, deductible status, and any authorization requirements before you walk in.
The evaluation (60 minutes)
Dr. Spencer takes a focused history, watches how you move, and performs an orthopedic and neuromusculoskeletal exam. We test, not guess — every hypothesis is ruled in or out with physical exam or functional testing.
Diagnosis and plan of care
You leave the first visit with a working diagnosis, an estimate of how many visits we think you'll need, and a plan of care that names the goals and markers we'll track.
Treatment starts at visit one
You don't wait for a second appointment to get treated. Wherever possible, we begin manual therapy, targeted exercise, and home programming on day one.
Follow-up and progression
Treatment is progressive. If something isn't moving in the expected direction, we change it. If you're recovering faster than expected, we discharge you sooner — you don't keep coming once you're well.

Dr. Spencer is the founding clinician of Forge Physical Therapy. He holds a Doctor of Physical Therapy and is a Certified Vestibular Specialist through the Institute of Advanced Musculoskeletal Treatments.
His practice is built around a simple idea: a boutique, clinician-led clinic where time, attention, and plan design are calibrated to the patient — so treatment is personal, progressive, and actually moves the needle.

Do I need a referral to see a physical therapist in Texas?
In Texas, you do not need a physician referral for a physical therapy evaluation. Direct access allows a licensed physical therapist to evaluate and begin treatment. Some insurance plans require a referral for reimbursement — we will verify your specific plan before your first visit.
How long is a physical therapy session at Forge?

What insurance do you accept?
Forge Physical Therapy is in-network with Blue Cross Blue Shield, Cigna, and UnitedHealthcare. Medicare credentialing is in progress — please contact us if you have Medicare and we will notify you as soon as we are in-network.
What should I bring to my first appointment?
Bring your insurance card, a photo ID, any imaging or prior medical records related to your condition, and comfortable clothing you can move in. If you have a physician referral or prescription, bring that as well — it is not always required but is helpful.
How many visits will I need?
Plans of care are specific to the individual. Most orthopedic conditions respond in 6–12 visits; some respond faster, and more complex cases may require more time. Dr. Spencer will give you a clear estimate at your evaluation and will revise it based on how you progress.
