Why Forge

Built differently — on purpose

01

Doctor-led, boutique by design

You see a Doctor of Physical Therapy every visit — not a rotating cast of techs. Our care model is deliberately staggered (never more than two patients at a time), so evaluation, treatment, and progression all happen under the same clinician's attention.

02

Specialized, not generalist

Vestibular therapy, concussion rehab, and post-surgical care aren't add-ons here. They're diagnosed and treated by a Certified Vestibular Specialist using evidence-based protocols — not repurposed orthopedic exercises.

03

In-network, without the mill

We're in-network with BCBS, Cigna, and UHC. You get the insurance coverage you expect — inside a clinic built around outcomes, not throughput. Medicare credentialing is in progress.

Your clinician

Dr. Eric Spencer, PT, DPT

Dr. Spencer is the founding clinician of Forge Physical Therapy. He holds a Doctor of Physical Therapy and is a Certified Vestibular Specialist through the Institute of Advanced Musculoskeletal Treatments.

His practice is built around a simple idea: a boutique, clinician-led clinic where time, attention, and plan design are calibrated to the patient — so treatment is personal, progressive, and actually moves the needle.

In-network with BCBS, Cigna, and UHC. We verify your benefits before your first visit — so you know your expected copay, deductible status, and visit authorization in writing. Medicare credentialing is in progress and we'll update this page the moment it's active. Questions about coverage? Call (214) 774-0600 or email billing.

FAQ

Frequently asked questions

Do I need a referral to start physical therapy in Texas?

No. Texas has direct access, which means a licensed physical therapist can evaluate and treat you without a physician referral for most conditions. Your insurance plan may still require a referral for reimbursement — we verify that before your first visit so there are no billing surprises.

Is Forge in-network with my insurance?

Forge is in-network with Blue Cross Blue Shield, Cigna, and United Healthcare. Medicare credentialing is in progress. For any other carrier, we can often still see you with out-of-network benefits or a cash-pay rate — call (214) 774-0600 and we'll run a benefits check before your first visit.

How is Forge different from other PT clinics in Frisco?

Most clinics run a high-volume, multi-patient model where a tech runs exercises while the physical therapist floats between rooms. Forge is built the other way: a Doctor of Physical Therapy manages your care start to finish, and we deliberately limit how many patients are scheduled at once so evaluation, treatment, and progression happen under the same clinician's attention. You are not handed off.

What should I expect at my first visit?

Your first visit is a 60-minute evaluation with Dr. Spencer. Expect a focused history, a movement and orthopedic exam specific to your condition, and — in most cases — a working diagnosis and a starting treatment that same day. You leave with a plan, not a follow-up appointment to get one.

How many visits will I need?

It depends on the condition. Some cases (uncomplicated BPPV, acute neck or back episodes) resolve in one to three visits. Post-surgical rehab or chronic conditions run longer. At your evaluation, Dr. Spencer will give you an honest, evidence-based estimate — not an open-ended prescription.

Where are you located?

Forge Physical Therapy is at 2650 King Road, Suite 500, Frisco, TX 75036. We serve Frisco, Little Elm, Prosper, The Colony, and surrounding DFW communities. Free parking at the door.
